New Client Information Form

This form allows us to record owner and pet information into our management software so that we can effectively and efficiently provide the service you deserve!  Please download, fill out and bring this form with you on your first visit to save you time.

Pre-op Lab and Pain Control Form

If your pet is scheduled for an anesthetic procedure, such as surgery or a dental cleaning, this form gives us permission to determine the current state of your pet's health via lab work and/or if indicated administer pain medication..

This form explains our boarding policy.  If your pet will be boarding with us, please download this form and bring it with you when you drop off your pet.

This form explains our bathing policy.  If your pet will be receiving a bath at our facility, please download this form and bring it with you when you drop off your pet.

This form lists the vaccines, lab tests on stool samples, dewormings, and microchipping routinely recommended for city puppies beginning at 7 weeks of age.  If you have a new puppy, download this form and bring it with you.  We will tailor it to your individual puppy's needs depending on age, previous vaccinations, life-style, etc.  We then bundle those services into a package that can be purchased on your first visit that affords you a 15% discount on the package.
